The ages of birth through 5 years are critical times in your young child’s brain development and a great time to support early learning. 

Use your child’s questionnaire results as guides for skills and behaviors that are expected at this age. Review your child's results with caregivers and discuss ways to follow up together.

Share Your Child's Summary

Share your child’s Summary, strengths, and any concerns, with your healthcare or childcare provider and teacher. Review the Summary and discuss ways to follow up together. 

Your child’s Summary is not automatically sent to anyone, so you decide if you would like to share. Here’s how:

To share your child’s Summary by email:

  • Email the Summary to yourself
  • From your email, choose Forward, enter the recipient
  • Make sure the Summary PDF is attached
  • In the body of the email, write something so they know why you are sending it
  • Send

Other ways to share:

  • Download and print the Summary PDF or paper copy
  • Take screenshots of your Summary PDF and text from your phone/device

Reviewing your child’s Summary with your healthcare or childcare provider is a great conversation starter. Help them get to know your child, your family routines, and goals.

Catch Concerns Early

When follow-up and support can make a difference.

You can use OSP to check your child’s development every 4 to 6 months. Continue to practice skills and check progress.

If you have concerns at any time, contact your local Oregon Early Intervention provider. Talk with a developmental specialist and request a free development checkup. Let them know your child's ASQ and ASQ:SE results and any concerns you may have. You do not need a professional referral.

What is Early Intervention?

Early Intervention offers free developmental services for eligible children, ages birth to five years, and their families. These may include home visits with a developmental specialist or services such as speech therapy.
